Englewood Department of Health does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service -- VitalChek Network, Inc. VitalChek can be reached through its website, www.vitalchek.com. An additional fee is charged by VitalChek for using this service, and major credit cards are accepted, including American Express®, Discover® or MasterCard®. Visa® debit cards are accepted in some cases.

The service window for vital records and licensing is open 9:00 a.m. until 4:00 p.m.

Copies of Vital Records

Birth, Marriage, Civil Union, Domestic Partnership, and Death Certificates are available from the Englewood Department of Health if the event occurred in Englewood. State Statute requires that the person requesting Certified Copies of these records must know where and when, as well as other identifying information, and also establish "right to access" to the record with identification. A Certification may be provided to persons without right to access to certified copies (not valid for legal business purposes), but the identifying information must still be provided. Only a record that qualifies as Genealogical may be searched for without the exact information otherwise necessary.

As of November 1, 2011, per Board of Health Ordinance 11-03, the fee for all Certified Copies and Certifications is $25.00 for the first copy of a vital record, and $2.00 for each additional copy of the same record, and the fee for amendments/corrections to vital records is $20.00No Personal Checks are accepted for these documents.

Mail Orders must be accompanied by a Money Order, Funeral Director's or Attorney's Business Check, or Certified Check.

Credit Card Orders are currently accepted through VitalChek
(order processing fee of $7.00 per order) with Expedited Delivery Service available by selecting UPS
Additional Credit/Debit Card Service, at a modest fee to the customer, is available at the Customer Service Window (Point-of-Service).

IMPORTANT INFORMATION ABOUT NEWBORN BIRTH CERTIFICATES

Please Note: Allow 4 to 6 weeks processing time (for hospital and Health Department) for Birth Certificates of newborns. The Health Department Vital Records Unit has no provision for expedited processing.

Marriage & Civil Union Licenses

State of New Jersey guidelines and application forms

Applications for Marriage or Civil Union Licenses must be made with the Registrar's office at least 72 hours in advance of the time the couple expects to pick-up the license.

Applications are taken by appointment from 9:30 a.m. to 11:00 a.m. or by special arrangement with the Registrar, Deputy Registrar or Alternate Deputy Registrar(s)

(Please note that personal arrangements for your Ceremony must be made either with Clergy or a Mayor or a Judge)

Domestic Partnerships

Affidavits of Domestic Partnership are taken by appointment from 9:30 a.m. to 11:00 a.m. or by special arrangement with the Registrar, Deputy Registrar or Alternate Deputy Registrar(s)

Pet Licenses & Owner Guidelines

Retail License Guidelines

Revised Fees effective as of November 1, 2011

(refer to Ord. 11-02 revising Chapter 6 of Health Code “Fees & Fee Schedule”)

Plan Review Applications are available below.

All new retail applications and fees must be reviewed and approved by a Health Department inspector (REHS) prior to processing by the License Clerk. 

Temporary License Applications for special events must be completed and submitted with payment at least five (5) business days prior to the scheduled event. Any event scheduled outside normal business hours (9:00 a.m. to 5:00 p.m.) will be subject to an inspection fee regardless of the license status or related fees. Effective November 1, 2011, ALL applicants must submit a $60.00 setup deposit to cover the cost of inspector's wait time if not ready for inspection at the time specified/agreed to. (Deposit to be refunded if applicant is ready for inspection on time.)
