Englewood Department of Health does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. Copies of Vital Records
Birth, Marriage, Civil Union, Domestic Partnership, and Death Certificates are available from the Englewood Department of Health if the event occurred in Englewood. State Statute requires that the person requesting Certified Copies of these records must know where and when, as well as other identifying information, and also establish “right to access” to the record with identification. A Certification may be provided to persons without right to access to certified copies (not valid for legal business purposes), but the identifying information must still be provided. Only a record that qualifies as Genealogical may be searched for without the exact information otherwise necessary.

As of June 1, 2006, the fee for all Certified Copies and Certifications,
for the Englewood Department of Health, is $15.00 each.
No Personal Checks are accepted for these documents.
Mail Orders must be accompanied by a Money Order, Funeral Director’s or Attorney's Business Check, or Certified Check.
Credit Card Orders are currently accepted through VitalChek

Please Note: Allow 4 to 6 week processing time (for hospital and Health Department) for Birth Certificates of newborns. The Health Department Vital Records Unit has no provision for expedited processing.

Applications for Marriage or Civil Union Licenses must be made with the Registrar’s office at least 72 hours in advance of the time the couple expects to pick-up the license.
Applications are taken by appointment from 9:30 a.m. to 11:00 a.m.
Or by special arrangement with the Registrar
(Please note that personal arrangements for your Ceremony must be made
either with Clergy or a Mayor or a Judge)
